Local 12 is always looking for programs that might be benificial to the members
of Local 12. Local 12 realizes that not all members will agree on every program
we offer to members. Offering these different programs to dues paying members
does not constitute an endorsement by Local 12 of the service being provided,
just that these are services that you as a member might want to use.
Dental Plans - Local 12 offers two different dental plans that you can join at
any time instead of waiting for the health insurance open season, The Standard
and Willamette Dental.
Willamette Dental is a HMO style dental plan where you use their doctors in
their dental offices. Willamette is only located in Washington, Oregon,
and Idaho so if you are outside of that area this plan would not be for you.
This plan that is managed by Local 12 without a third party broker. Click

rates shown are monthly rates. However, as the Willamette plan is
contracted through Local 12, the premiums are a payroll deduction and would be
reflected on your LOEP in the block :Union Dues." The bi-weekly premiums
would be, self only $20.17, for self and one dependant $39.54, and for
family $58.11.
The Standard plan is managed for Local 12 by a third party insurance broker,
CAMCO. With this plan you go to your own dentist and the plan will pay a
percentage of the work you have done based on the type (low or high option) of
coverage you elect to have. To see this plan, click

They have also added a plan for people who have need of braces or implants. Click

Applications for these plans are located in Local 12's office in Building 850
3rd floor, east end. Premiums for these plans are collected by CAMCO.
They use the direct debiting method and that form is also in the Local 12
office.
